Hi,

I'm glad you caught their mistake. Is there anyway to get with this doctor and have them give you a copy of what was put on your chart? I'd be concerned not only for the nurse's mistake, but if someone else's records might be in your chart, that could be a possibility also. I'd be very polite, but I'd want my records to be correct, and let the doctor know that. If you went to another doctor, you may need to transfer records over, and they'd see this.

I'm sorry this happened. Let us know what goes on.
