We often use higher doses of metformin up to 850mg twice daily. Unfortunately, the literature has not yet shown an equal benefit from Glucophage XR.

Best success will come when the Glucophage/metformin is combined with low processed carb diet and exercise. (see http://www.ivf.com/pcostreat.html)

Be carefull when adding clomiphene. If the LH:FSH ratio is elevated after clomiphene (day 8 if taken day 3-7), then results are diminished.

I would consider up to six months prior to initiating clomiphene (see http://www.ivf.com/clom.html) to limit the risk of multiple births and possibly miscarriage. Ovarian drilling may also have a place in managing PCOS in the non-smoker.
