Spironolactone is a medication of necessity for PCOS in the US. It is no real medication for PCOS or hyperandrogenism for that matter, but other antiandrogens (although available) are rarely prescribed in the US. But antiandrogen therapy only counters the preexisting high testosterone levels, but does nothing to reduce it. It is insulin sensitizers that solve the problem, which is an undeerlying metabolic disease (insulin resistance)
