Your information is right; about the only thing that can be determined from a pelvic (other than cervical cell abnormalities from the actual pap smear) and the ensuing external palpitations is a large fibroid or general swelling of the ovaries. But the swelling of the actual ovaries would have to be REALLY great in order to palpate and discover it.

The ultrasound to detect PCO cysts is usu. a vaginal one; the regular type of ultrasounds tend not to be so accurate in detecting the small nature of the cysts. A vaginal ultrasound isn't the most pleasant, nor is it the most comfortable, but I've definitely had *more* discomfort. It's not the most fun I've ever had at a doctor's office, but it's not the worst experience of my life, either.

Some cysts are so small and close together that even on a vaginal US they show up only as "ridges." I've been told that the only way to REALLY confirm such types of cysts (I have that kind) is to go in via abdominal surgery and actually palpate the ovaries by hand.
