am sorry to hear you are having such a hard time and that you have not been given much information.

What medications are you taking? Are you taking meds or insulin for the diabetes, pcos, high cholesterol, blood pressure?

You need to have meticulous control of your blood sugar and agressive treatment / management of the PCOS and diabetes.

Have you been tested for low thyroid. This can also contribute to all the problems you mentioned.

Also, you will need to have a lifestyle management approach. This means exercise (as much as you can manage and as intensive as you can manage, you may need to go for low impact due to your weight, but you must do it).

Diet management is critical as well. Cut out all saturated fats and watch your cholesterol intake. Eat healthy fats (fish oil, avocado, etc). Cut way down on / elimiate any refined carbohydrates or highly processed foods. This means your diet will consist mainly of low fat protein and low glycaemic index vegetables. Some dairy is fine. Again, meticulous monitoring and control of your blood sugar will be critical.

Sorry there is no easy way to manage this. What you need to do now is learn as much as possible and focus on getting the best possible management. BTW, there is a reasonable chance that you can conceive, so don't worry unduly about that for now, instead focus on getting healthy.
