WESTPORT, Nov 29 (Reuters Health) - Color flow Doppler ultrasound appears to be an effective tool for planning management strategies in patients with early invasive cervical carcinoma. Taiwanese investigators in Taipei followed 104 women diagnosed with stage IB-IIA cervical carcinoma. Prior to hysterectomy and pelvic lymph node dissection, Dr. Wen-Fang Cheng and colleagues used transvaginal color Doppler in this study group "...to search for arterial blood flow signals within the tumours." The researchers from the National Taiwan University Hospital discovered that 58% of the tumours had detectable blood flow signals. The characteristics of these signals in the cervical carcinoma patients were "...associated with tumour angiogenesis and could reflect the likelihood of parametrial invasion and lymph node metastases," the Taiwanese team writes in the November issue of the British Journal of Obstetrics and Gynaecology. Detectable intra-tumoural blood flow correlated with larger tumour size, "...deeper stromal invasion, invasion of the parametrium, pelvic lymph node metastasis and greater microvessel density." Dr. Cheng's team suspects that angiogenesis is reflective of "...the local invasive and metastatic abilities of the disease." Finally, absence of intratumoural blood flow suggests to the authors the absence of parametrial invasion and pelvic lymph node metastases. Br J Obstet Gynaecol 1999;106:1137-1142
