We have a patient with a history of tubal ligation. She now presents with a Grade 2 endometrioid cancer (<5% sheet like growth, but focal grade 3 cytologic features). The tumor grew in a superficial/spreading pattern, with 0.4 cm invasion of a total wall thickness of 2 cm. However, the tumor grew along the epithelial surface to involve the upper endocervical canal, and also spread superiorly to involve the epithelial surface of a proximal fallopian tube stump. All margins, including the stump, were negative. We are staging her as at least a IIA (cervical gland involvment), but the question is should she be upstaged to a IIIA based on the fallopian tube involvment? If so, would she still be expected to behave more as a IIA, since there was no path for spread from the fallopian stump?
