How about inguinal endometriosis? Another possibility is a noncommunicating uterine horn, although this would be surprising since she has had surgical evaluation of her uterus already. You could try a trial of amenorrhea, perhaps using a gonadotropin releasing hormone agonist, or surgical evaluation with laparoscopy.
