Here are some juicy bits from the latest Green Journal..

1) Tocolytics for Preterm Labor: A Systematic Review by Mary Hannah, etal (Gosh I wish we could get her on the list!). This paper reviewed the literature and found that tocolytics do prolong pregnancy but have not been shown to improve perinatal or neonatal outcomes. 2) LSR has a paper: Cervical Ripening and Lbaor Induction with a controlled-release dinoprostone vaginal insert: a meta-analysis Obstet Gynecol 1999;94:878-83. He seems to conclude that cervidil is not as good as other agents. (Not to mention it is really expensive, too...)

3) Gilmour, etal encourage us to cysto all patients after major gyn surgery.. I kinda like the idea. Have a look at: Lower Urinary tract injury during gynecologic surgery and its detection by intraoperative cystoscopy. Obstet Gynecol 1999;94:883-9.

4) ACOG came out with the recs for misoprostol use.. Hurray!!! See Committee Opinion #228 Induction of Labor with Misoprostol and Practice Bulletin 217 Induction of Labor. Looks like they lean toward the 25 µg dose every 3-6 hrs...

5) ACOG is ok with HRT in breast cancer patients. See Committee Opinion 226 Hormone Replacement Therapy in Women with previously treated breast cancer.

6) Combination Antiretroviral Therapy in human immunodeficiency virus-infected pregnant women by McGowan, etal Obstet Gynecol 1999;94:641-6. Seems that it won't be long before protease inhibitors are added to the protocol.

7) Dr. Stanford (congrats) tells us that we are not up to speed on natural family planning. Physicians' Knowledge and Practices regarding natural family planning Obstet Gynecol 1999;94:672-8. I must admit I am guilty as charged :(

8) Star, etal take exception with ACOG's position on cesarean delivery for prevention of vertical transmission of HIV. Should women with human immunodeficiency virus be delivered by cesarean? Obstet Gynecol 1999;94:799-801.
