If, on speculum exam, you see fluid running from the cervical os, then there is either ruptured amnion or a vesico-uterine fistula. The rarity of the latter, especially presenting anew in the third trimester usually is sufficient to rule it out.

Actually, I have seen an effort at ferning which failed during the 9th month of my G2Ppre1 wife. Unfortunately the amnion had probably ruptured and self-repaired because the baby was born with GBS about 96 hours later. Ferning, I think, is an imperfect assay. No paper was around at that particular office at that particular time, but the fluid leak was running clear and not ferning. Of course, Dr. Ramirez is right that that was my single experience. -Bert
