I had to "terminate" a patient a year or so ago when we could not agree on terms under which she would be able to continue receiving prescriptions for controlled substances. Our clinic's attorney told me that there is a general duty not to abandon a patient, but no clear-cut definition of what, exactly, that entails. She said that it is customary to give 30 days, but that nothing puts such a time frame in writing, let alon in stone.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
