Re : Is C/S the safest method of delivery?

Some times ago a colleague in my group had a G2P2, with a previous CS and an obstructing anterior placenta previa. Scheduled CS with another ObGyn surgeon, blood in the Op room... she developped an IDC while she still was in the post op room and she died... after the re-operation (hypogastric ligation + hysterectomy). Despite 105 minutes of external cardiac massage, 15 electric schocks and litres of fresh plasma. I know this is not a direct consequence of this CS but wasn't the first one some kind of cause for this anterior placenta previa?
