good question. the rates are as follows 1990 total 23% primary 16% 1994 total 21% primary 15% 1995 total 20-21% primary 15% 1996 total 20% primary 15%

there is some regional variation highest in south-- 21-26% lowest northwest-- 15-19% midwest -- 18-21% there is a good discussion of this in ob/gyn clinics june 1999 hope this is helpful gaetano venezia
