ok - here's an anecdote. patient is a 51 yr cancer-phobe who persuades her gyn to send off a CA 125. At about 1yr post menopause, her level is 50. TVS reveals no masses. CA125 repeated 3 mos later is 1000! repeat scan again no masses, but now moderate amt of free pelvic fluid noted. w/u essentially negative and pt has laparoscopy at which time disseminated intraperitoneal carcinomatosis is noted with ovaries normal save for external implants. path - papillary serous carcinoma - primary?.
