Several years ago an obstetrician I worked with left a tail sponge deep in the vaginal vault. The delivery was complicated by postpartum hemorhage. He also had done a forceps delivery and had to repair a 4th degreee laceration.

Unfortunately it was not discovered for about 10 days when the patient presented with acute sepsis and a huge abcess. She ended up with a hysterectomy. She sued (and settled out of court).
