definitely NOT standard of care and technically trickier than it sounds. also, it's not totally clear whether to then perform standard down's serum screening in second trimester if first trimester screen is normal. patients should be informed prior to the test that insurance companies may very well NOT reimburse follow-up invasive prenatal diagnostic testing should the NT suggest increased risk.
