12th April, 1999 Dear colleagues, May I ask for your advice about a patient I have come across recently. She is a 25-year-old woman pregnant with her first child. She is now 15 weeks pregnant. Three weeks ago (at 12+weeks of gestation) she swallowed hydrochloric acid in a suicide attempt. She recovered with conservative measures, but developed persistent vomiting after two weeks. An upper G.I. endoscopy today revealed extensive ulceration in the stomach and most importantly, PYLORIC STENOSIS that may well explain the vomiting. The gastro-enterologist is starting her on Omeprazole and anti-acid drugs but feels that she may well need a gastro-jejunostomy in the near future. Does any one have any experience in such a procedure in pregnancy and what is the prognosis for the patient and the child? Though some people are advising to terminate the pregnancy first, I am inclined to let the pregnancy continue and let her have the surgery (should the need arise) and await events. Please guide me. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
