For what it is worth, IMHO, adenomyosis is the primary cause of ablation failure. My current approach is if menorrhagia unresponsive to medical Rx, and if bleeding associated with severe cramps, and uterus tender /boggy/ marginally enlarged, I encourage LSH . A review of our first 55 cases of LSH, seven were ablation failures. Two were fibroid related, the others all adenomyosis.
