Patient satisfaction survey: Sample questions When arranging the appointment and entering the clinic ... It was easy to call the clinic and schedule an appointment. The time needed to schedule ahead for an appointment was what I expected. The waiting time in the clinic before the appointment was what I expected. When I was seen by the medical staff ... The MD/CNM/PA was willing to listen and answer my questions. The physician and CNM/PA communicated well to each other. I received an adequate explanation and information about my condition and its treatment. After my exam was finished ... The charges were what I expected. I understood the clinic billing procedures. The billing staff was helpful and knowledgeable about my insurance forms and questions. Overall ... I was satisfied that I received the most value for my time and money. I was treated with dignity and respect by the staff. I will recommend my clinician to family and others. I would like to see the following changes and improvements (briefly explain).

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
