Formaldehyde error

This story was broadcast on CBC radio "As It Happpens" on March 25/99. "Hannah Dolamore has only been alive for four months, but every day has been a struggle. A solution of formaldehyde was accidentaly poured into her vapourizer while she was in the hospital. Yesterday, the province's health committee launched an investigation into the case. Hannah's mother, Jessica Dolamore, is in Lethbridge Alberta." Apparently, Hannah was receiving phototherapy for jaundice. The nurse poured a bottle of l0% formaldehyde into the vapourizer on the incubator. She mistook it for a bottle of water. Hannah's skin was burned bright red and at four months old she requires oxygen at home 24 hrs/day.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
