I don't have them at my fingertips. I know the OBGYN "green" journal had a piece on authorship a while back. Also JAMA did, too, if I recall. I was drawing on my recent memory of same, but not specifics... Also, probably some of my ethical overlay from my own experience with publishing (and perishing).

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
