I would counsel this woman on diet, talk to her about use of warm compresses/use of tub during labor to warm the tissue and have her do small pushes with good perieneal support. I think that it is very important to give these women a positive message about their bodies ability to give birth. Their is a great book with a chapter dedicated to perineal health, massage and preparation prior to labor it is call Mother Massage and is available at most bookstores in the pregnancy section.
