A transvaginal ultrasound to check endometrial thickness could help. If the endometrium is found to be thick and cannot be sampled then one may consider hysterectomy. Serial ultrasound may be indicated if endometrium is thin and spotting recurs. However the possibility of endometrial cancer is quite low if you have been on adequate amount of progestogen while on HRT. Sometimes endometrial cell sampling is possible with ultrasound guided needle through the uterus but I have no data as to how adequate this might be.
