If the patient is an appropriate surgical candidate, I would perform radical hysterectomy and pelvic lymphadenectomies (now). If poor surgical risk (primarily marked obesity in this age group), would recommend abortion + radiation therapy. If patient refuses treatment that would result in loss of the pregnancy....would do Cesarean radical hyst and nodes once fetal lung maturity can be documented.
