I had an interesting case last pm and would like to know if anyone has seen something similiar. I admitted an 18y/o g1 in labor and took her to the OR for a stat C/S due to a tetanic ctx, and a drop in FHTs to the 80's without response to Terbutaline. I really thought I'd see an abruption since she smokes at least a 1/2 pack a day. Instead, upon entering the abdomen, I encountered the uterus in a more twisted fashion than expected. The left adnexa was in the midline. I made my LUT incision as best as I could, and delivered the baby with Apgars 8/9, no nuchal cord or other anomaly. Closing the hysterotomy was normal midline LUT. I can't explain the big ctx she had by what I saw other the twist. Is this the cause? Or am I following the rule of being lucky rather than smart? Oh yes, the C/S was done decision to incision less than 15 minutes..not bad for a community hospital in SD. Dan Flaherty MD, Watertown SD

The American Medical Association is no longer designating CME hours for AMA Category II CME credit.
