A 25 year old nulliparous lady, married 6 months ago presented with complaints (by the husband) of difficulty during intercourse due to an intravaginal mass obstructing. On examination, the cervix was pushed to the right side; the vagina was filled with a mass of size 6cmx6cm. Ultrasound followed by CT scan confirmed the presence of a broad lig.fibroid close to the cervix. No separate line of demarcation seen between the cervix and the fibroid. As the lady is nulliparous, it was feared that surgical removal of the fibroid had the risk of culminating in a hysterectomy due to possible injury to uterine vessels, she was advised to wait for spontaneous pregnancy failing which myomectomy was the plan. The lady conceived,went through the pregnancy punctuated by abd.pain(? due to red degeneration of the fibroid), and was delivered by LSCS at term. Operative findings: Live baby 3.85Kg, Fibroid 8x8 cm in the left fibroid ligament pushing the uterus to the right & up. Was it correct to allow the patient to get pregnant before myomectomy? Patient wishes to have one more baby before myomectomy. Is it OK? Please opine. Thanks.
