Fetus died within last week, EFW 3500-4000g, Waters I have been the unwilling recipient of several back down transverse lies in residency and already had one since I've been out in practice a month now. One of my least favorite things :( I have managed to deliver a couple by conversion to breech extraction and one to vertex by making a lower segment incision and then rotating baby. The biggest problem with this is that sometimes you are limited by fear of harming baby either by direct trauma or by delay in delivery. I would think with a dead fetus you could be much more successful in this regard. Having said this I would strongly consider an attempt at external version (with epidural) and then induction if you can achieve either breech or vertex presentation - do an amnioinfusion if unsuccessful. Interesting case. Please give us followup on outcome.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
