Placenta Praevia

Forgive me if this has already been discussed but it is the topic of some debate in our unit at present.

How do members of the list believe placenta praevia prior to delivery is best managed firstly in women in the third trimester who are asymptomatic(ie no bleeding at all) but who have placenta praevia picked up on a scan for something unrelated which is otherwise reassuring about fetal health.

secondly in women who have a small bleed but then settle.

Does the grade of placenta praevia affect your management.

I am particularly interested to know about the decision for inpatient or outpatient care and if outpatient what monitoring you would have in place.

My searching of the literature is that very little has been done comparing inpatient and outpatient care and the total number of patients in randomised studies that I have found is less than 200. Can anyone pooint me in a more informed direction.
