This can occur in women with cycle less than 26 days - so that ovulation is before day 12 - so that with 5 day menses + 7 days after conception can not occur. It can be a difficult problem to treat - sometimes clomiphene will lengthen the cycle enough to delay ovulation past day 12, and now with GnRH it is very easy to do. There have been rabbinic responsa on the issue - don't have any to hand at present.
