Quite agree that the message was that the excess mortality was due to home births in high risk cases. The paper therefore has little or no relevance to our local experience of home births, where it is restricted to low risk cases. >

The BMJ has a very strict linguistic approach which involves a control of style and spelling in the BMJ, its associated journals and other in- house publications. I have even had attempts to change language in a medical book for lay-readers, I wrote, which was published by a company formerly associated with the BMJ. Fetal is definitely the accepted BMJ house style. It is of course correct English usage. Foetal is rarely seen even in England.
