We treat all bleeds w/ rhogam, but use the 50 µg dose in first trimester.

At which gestational age do you switch to the 300 µg dose (assuming minimal bleeding), and how frequently will you repeat the dose if they are bleeding? Theoretically, a 300µg dose of rhIG should be protective at least 12 weeks (the rationale for giving it prophylactically at 28 weeks), but I have been surprised how quickly the antibody screen reverts to negative after administration.

Do you ever check Kleihauer-Betke antepartally, or just give 300µg whenever the woman bleeds?
