I'm unclear from your post whether or not the uterus was "T'd". If so, that needs to be managed as a prior classical C/S.

Usually when I've assisted and we've had the difficulty you describe, the incision is enlarged horizonally. The only two instances I've seen the uterus T'd is when there's been a transverse lie. Both times the obstetricians wished they'd started with a vertical incision.

Unless you can get an op report indicating no vertical uterine incision, I think the patient should have a repeat C/S. I'm sure all the OBs I've worked with would agree.
