He basically got them to feel how much of the head was palpable abdominally. As an aid it was recognised that a term head was about five fingers breadth from bottom to top. So three fingers breadth palpable was 3/5. They weren't midwives they were lay-carers. >

I have no great experience in this population (el has!).

However my understanding is that it is largely to do with a combination of sacralisation of L5 and a so-called "high inclination" pelvis.
