I was a resident assisting an experienced (60 Y/O) private staff. The patient weighed 290 pounds. We did not do glucolas in those days and there was no ultrasound. He applied the forceps at +3/5 station, then pulled and pulled and pulled. then he put his feet on the table and pulled and his face turned blue and I thought he was going to have a heart attack and then finally the huge fat face came out and turtled on us. This totally unflappable staff man then giggled in a high pitched giggle that he had and said "Oh Shit!!!!" Never heard him say that before or since. Then there was the suprapubic pressure, the Woods screw maneuver, and finally the successful delivery of the posterior arm. The child spent the next two weeks laying on a crib in the "Premature Nursery" It had dificulty moving both arms on discharge from the hospital. I ahve no idea of the long term outcome. Dan

Was the kid OK? How about the mom? How about the doctor?
