Perform a cerclage or not?

Hello all of you. I have a new case for the list

Patient 38 yo P4G2 two normal deliveries at 1981, get pregnant 1 year before lost her baby after amnionitis occurred 1 week past amniocentesis

at 20 w, .

Now pregnant again 27w , refused amnio but all seems to be ok for the baby The problem is at the cervix where i noticed in vaginal u/s 3 days ago an 15mm dilatation in hole length . In a sterile speculum examination i saw membranes protruding external cervical os

u/s findings 12w crx length 37mm totally closed 20w length 35mm 5mm dilatation 24w length 30mm 5mm dilatation now 27w length 30mm 15mm dilatation

No contraction at all without any tocolytic agent What will be the most appropriate management after these findings To perform a cerclage will be dangerous?
