Endometrial cancer staging following vaginal hysterectomy

I am a primary care physician caring for a postmenopausal woman s/p vaginal hysterectomy without BSO for endometrial dysplasia, with pathology showing stage IA, G1 adenocarcinoma. Her gynecologist is now recommending laparoscopic BSO to complete staging, but the patient has asked me whether this is really necessary. She is concerned about the small but finite risk associated with the procedure in light of the small likelihood of involvement of her adnexa. I have been unable to find anything in the literature that really addresses this situation. Is removal of the adnexa really necessary?

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
