Dear colleagues, I cannot find much about BDA and pregnancy, can you help me? Patient, 24 yo, G2, one year ago intrauterine fetal demise at 28 wks with symmetric IUGR, 580 gr, male, prominent notching of uterine arteries on doppler ultrasound. Pathological postmortem exam: placental insufficiency, no anomalies on fetus. DBA diagnosed at 1 yo, normal kariotype. Unsuccessful kariotyping of fetus. Now she is 27 wks pregnant, symmetric IUGR from 20 wks, now fetus size as for 22 wks of pregnancy. No growth of fetus any more, reverse flow of a. umbilicalis, prominent notching of uterine arteries, oligohydramnion, female, "fibrous placenta". In both pregnancies she received blood transfusions at Hb under 80 g/l to have this value above this. Unsuccessful therapy if IUGR with acetilsalic. acid, nonsaturated fat acids, decompressions, bed rest in actual pregnancy. More or less typical early symmetric IUGR. Could it be linked to Blackfan-Diamond anaemia? We plan to do kariotyping of fetus and placenta and bone marrow biopsy at birth of this fetus. What else?
