Hi Lenora. I would use MSS not AFP. If a 40 yr. old has a Down's risk of 1:800, her risk for a Down's is 1:800. I think the patient is correct in the way she would use the information. MSS doesn't substitute for amnio as you've said. It is another bit of information that she can use to determine if she's comfortable taking a chance losing the pregnancy.
