Re: Acute Pyelonefritis and lombar/pelvic painFrom: Ricardo Savaris (savaris@orion.ufrgs.br)Thu May 7 10:26:16 1998
Dear all, thanks for your replies, I saw the patient this morning. She looks fine, very active, still complaining about her pain, but not that much, she can cope with it. If she retains urine for a longer time, her low abdomem distends. The TVUS was dead normal, only a folicular cyst on right ovary, no myomas. I became very concerned about myself, since I did the vaginal exam during admission and did not find anything, and then comes the radiologist telling me about a pelvic mass... She is not having fever for the last 3 days, her pain diminishes with sodium diclofenac There are no stones on the urinary tract, (the CT scan was performed using contrast, and it was clear all way down, from the kidney until the bladder, the same about the ultrasound). My collegue urologist suggested to give valium bid, add sodium diclofenac, send her home and ask for an uroculture few days later. The nephrologist said to send her home with the same antibiotics to complete 14 days, after that, wait 2 days and do the uroculture. She also said that the pain is very comon in this situation and probably persists longer than the end of the treatment. So I will dischrge from hospital today, and I will give some feed back when she returns. Thanks
