Fortunately, these cases are few and far between. We had such a patient while I was Chief Resident at Bellevue in 1979. After reviewing the literature at that time, we concluded that the risk of maternal mortality with portal hypertension with varices was 20% - worse odds than with Russian roulette. The woman terminated that pregnancy.
