I place them on estrogen when they're ambulatory. Of particular note: I do NOT believe that a "one-size-fits-all" estrogen dose is appropriate. It's amazing how many VERY YOUNG (35 - 40ish) women are given the patch when their estrogen needs are dramatically greater than the older 60ish menopausal woman. I, therefore, have been using a low dose OC in these patients (Lo Estrin 1/20 works very well) for their long-term HRT regimen. It works dramatically better than the lower dose patches. I usually start after they're home and ambulating very well, I do NOT start the OCs in the hospital while the patient is still significantly immobile.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
