GYN: Breast self examination cards

For years, my practice has purchased plastic cards to hang from the shower with BSE instructions. Of course, there is a place to have your practice name printed on it. Unfortunately, we need more cards, and no one can find an invoice, etc. from the manufacturer, and the card doesn't have any useful information on it.

Does anyone have a source?

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
