In my office, I do vag exams at the first office visit and then again whenever there is an indication. Once the patient arrives at the hospital, it is exams as needed to follow her labor. I have not done routine weekly exams after 36 weeks, since I stuck my finger through a 36 week placenta previa that had had no prior bleeding.
