Answers:

Monthly to 28, every 2 weeks to 36, then weekly to 41. Usually I induce before 42.

>2. Do you perform a vaginal exam at each OB visit?

No. At initial New OB Visit, then again if symptoms warrant. At 36wks, vaginal strep culture, then I usually check dilation once after that, especially if patient has symptoms of labor, lives far away, or I'm unclear about presentation. At 41 weeks I'll check dilation prior to induction.

>3. How do you control the evolution of a delivery? Vaginal exam or not?

I'll examine patient when admitted to L and D. Usually again when pt. requesting something for pain management...prior to epidural or narcotics. Again when pt begins pushing. Most of the time I keep my examining hands off during second stage until the very end...
