Dr. Waldby gives an excellent summary of what is necessary to treat *invasive* pseudomonas disease (i.e. pneumonia). The important question is to ask yourself what the pseudomonas represents. Is it a chronically draining sinus or fistula? Or is it simply colonizing a moist dark place? Chronic external otitis with pseudomonas (actually is most common organism with fungi next) tends to respond only to topical treatment. We often use the opthalmic preparation of ciprofloxacin to good effect here.

I would agree with all previously mentioned efforts to make the vaginal ecology more hostile to the organism. In addition, this is one situation where frequent douching with an antiseptic solution might well be the most effective treatment (barring evidence of necrotic focus or fistula). If nothing else is effective, consider topical use of a quinolone (I know of no commercial product, would likely have to be compounded at significant expense).
