With extensive condylomata of the vagina and cervix I have had limited
success using destrctive methods (excision, multiple Bx, electrocautery)
in the office (with local for vaginal lesions).  Multiple treatments
with TCA has not worked as well for me.  Best results achived by
agressive treatment in an OR with general anesthesia and laser and/or
excision.

I suspect that agressive destruction decreases the viral load giving the immune system a chance to do its thing.

Had a kidney transplant patient on immunosupressive drugs with condylomata of the labia. She had recurrance even after laser. Finally did skinning vulvectomy and skin graft. That has worked. I believe that this reflects the important role of the immune system. >
