I refer all circs to the urologist who is quite happy to do them. Our pediatricians have stopped doing them and only a handful of the OB's continue the practice. We have a vocal anti-circ group of nurses that make it difficult for the patients who want them. Therefore its easier to refer them to a private office where they can be performed on the biblical "8th" day. BTW, I've never understood the rationale behind removing every milimeter of foreskin. The Jewish moyels used to remove only about half. That way there remained some skin to protect the proximal glans, yet hygiene was improved since retraction was greatly facilitated by the operation.
