I want your opinion about a patient 24yo, primipara, no any problem till 27 w started a slide elevation BP 140/90. I made all lab test( no proteinouria, renal function ok, liver enzymes ok, PLT ok, no elevated Ca++, doppler at 22w and 27w ok, AFI ok, no IUGR, glucosa ok). With a-methyl-dopa 0,5 gr per day BP 110/70. The only i notice is a continues elevation of uric acid from 3,2 at the early pregnancy 3,8 at 20w 4,6 at 27w after 4 days 5,8. Now the pregnancy is 32 w and UA is 7,8. All other laboratory test are ok( every week check up) The only i can find at the u/s i made at 32 w is a subchorionic fluid collection, after a doppler determination seems to be a blood lake( no other u/s findings , s/d ok, AFI OK, no IUGR, BFP OK) How i can manage this pregnancy. What means this elevation of UA , any association with the u/s findings? Can this pregnancy continue till end or induce earlier? Any opinion about the u/s findings? Any help acceptable.
