Re: breech dilemma updateFrom: Marguerite H Epstein (mgtepstein@juno.com)Sun Mar 8 00:04:30 1998
No she was correct. The lie with the breech is longitudal, The presentation was breech, don't have postion because the placement of the denominator was not mentioned. Now that the baby turned, the lie is longitudal, the presentation is vertex, and again the postion is not determined 'cause the we don't know where the denominator is. This is a pet peeve of mine, I don't want to sound too bitchy but too many people mangle this stuff. With your indulgence I will define the terms. Lie: where the long axis of the baby is; longitudal, transverse, obligue (breeches and cephalic are both longitudal, shoulder is usually transverse) Attitude: the relationship of the head to the trunk; flexed, deflexed, extended Presentation: the fetal part in the lower pole of the uterus; vertex, breech, shoulder, face, brow. Vertex, face and brow are all CEPHALIC presentations (cephalic is a catagory of presentation). A flexed head is vertex. Denominator: The name of the part of the presentation used when refering to position; in the vertex it is the occiput, in the breech it is the sacrum, in the face it is the mentum, in the brow it is the bregma Position: the relationship between the denominator and points of the pelvis. So you were right and wrong at the same time. No a breech is not vertex, yes a vertex is cephalic. Again, sorry if it sounds bitchy Marguerite Epstein L.M. Licensed MIdwife Ft. Lauderdale, FL

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
