This is kind of a difficult question. Without knowing precervical length it might be difficult to determine if the patient is having clinically significant shortening. The problem is that the patient was treated with a cerclage the first pregnancy and if by some chance she has a loss or preterm labor I can bet you that her lawyer will be contacting you. I don't feel that it is standard of care to place a cerclage but with her prior history of having one you are in a difficult situation.
